Formato Basico Application Form
This is one of the application forms you need to fill out at the INM office when applying for almost all changes to your immigration status. It replaces the old FM1, Formato de Filiación.
It looks like this:
You can get it at the INM office or you can fill it out here and print it. In that PDF form if you scroll down there is an English translation. (thanks to Spencer). Printing it is a good idea so you have everything you need when you get to the INM office. They will ask you to enter the data into the form on a computer in the INM office so having it all with you is a big help (see comment below).

I agree with otherss about the usefullness of this page.
I would like to add that this electronic immigration system in Mexico is a hallmark and should be adopted by other countries worldwide.

I see we have a comment above that says you enter the data into a computer at the INM office. This year (2011) when I went to the Bucerias office I filled it out by hand at the counter. Can anyone tell us if they have used a computer for this? Perhaps the comment should be removed.?
Thank you for the translation into English it was very helpful. It made the form so much easier to fill out, that is with having a mexican person translate the Spanish as well. Hope you have a great day
